How to fill this form
Please enter your full name as you would like it to appear in the certificate, and use an email address you check regularly. Select an appropriate title (such as Dr, Ms, Mr, or leave it blank if you prefer) and indicate your team leader(s) you went with.
There are no automatic email confirmations; after you submit, you just get a pop up message.

Review your details carefully before submitting. If your email address is incorrect, we may not be able to contact you. After successful submission, you will just get a popup confirmion. Please note - This is only for Cluster 2 Volunteers. Those who did not cover Cluster 2 at all should contact their Team Leader for this purpose.
How we use your data
Your contact details are stored securely and are accessible only to authorised Delhi Bird Atlas organisers. We would use this information to share survey-related updates and your certificates if issued digitally. Your email address will not be displayed publicly or shared with other entities.
